“It was time to focus on me for a bit,” says Harry Ciaccio. Ciaccio, a Doylestown resident, had seen the “wonderful work” that had been done to his daughter’s teeth and decided there was no good reason he shouldn’t have the same.
When his daughter finished college, Ciaccio visited Bruce J. Wilderman, D.D.S., of Artistic Expressions Dentistry, an adult cosmetic and restorative dentistry practice, also in Doylestown. After learning his options, including the costs, timeline and other factors, he decided to invest in a “smile makeover” comprising multiple crowns, as well as solutions to address a few missing molars and gaps in his front teeth that had made him self-conscious for too long.
“I’m in management, and I felt my smile was starting to look pretty bad,” Ciaccio says. “I had felt uncomfortable about my teeth, and [Dr. Wilderman] made them look natural, like my teeth actually belonged in my mouth. It improved the aesthetics of my face and helped my overall image.”
Although many people opt for cosmetic procedures in order to land a new job or to earn a promotion as a means of enhancing one’s earning potential, Ciaccio suggests the greatest benefit is nearly impossible to quantify: “I’m not self-conscious about my smile anymore. It’s provided me with more confidence.”
He has plenty of company. Such means of self-improvement—often embraced by folks who are in the aftermath of some sort of major life change—have become a booming business. Why? Because looking good and feeling good go hand in hand, and feeling good about oneself, to Ciaccio’s point, is a priceless gift, according to Peter Flaherty, D.M.D., of the Wayne-based dentistry practice Main Line Smile.
“It can be as little as a shot in the arm to something that’s absolutely life altering, depending on the degree to which someone is suffering,” he says. “In having cosmetic work done, people no longer have to cover up their smile. Men are able to shave their mustaches off. … It can change lives, and that’s the really special part of dentistry.”
In addition, it seems cosmetic procedures just might be a good barometer of the U.S. economy.
“When everything crashed in 2008, cosmetic dentistry kind of vanished,” Dr. Flaherty says. “It’s gotten stronger and stronger in the years since; we’ve probably just had our busiest year since ’08, where [cosmetic dentistry] accounts for about 50 percent of the practice. … It’s almost like being in the stock market; you need to be diversified in case the economy goes under.”
The No. 1 “game changer” Dr. Flaherty has seen in cosmetic dentistry over the past 10 years has been the utilization of Invisalign. These clear, plastic orthodontic aligners enable patients of all ages to straighten their teeth without invasive drilling or even traditional wire-and-bracket orthodontics. Also, when utilized, complementary technologies such as AcceleDent—an FDA-approved treatment that features a mouthpiece with a built-in activator that uses micropulses to accelerate the movement of orthodontics-guided teeth—have trimmed average treatment times by as much as 50 percent.
In general, dentistry has been trending toward minimally invasive procedures—moving teeth rather than drilling teeth, whitening teeth rather than veneering teeth, treating conditions such as temporomandibular joint pain with injections of the neurotoxin Botox, etc.
Speaking of Botox, just like in dentistry, cosmetic medicine has been steadily moving toward minimally invasive or noninvasive procedures, such as laser skin rejuvenation and dermal fillers. That is not to suggest, however, that the demand for traditional plastic surgeries has declined. In 2013, the United States saw a 12 percent overall increase in surgical and nonsurgical cosmetic procedures—11 million procedures in total—that were performed by board-certified plastic surgeons, dermatologists and otolaryngologists, according to research from the New York-based American Society for Aesthetic Plastic Surgery.
This marked the first time since 2008 that Americans spent more than $12 billion on cosmetic medicine. Of that amount, more than $7 billion was spent on surgical procedures (the top five, in order, were liposuction, breast augmentation, eyelid surgery, tummy tuck and nose surgery), with the remaining $5 billion spent on nonsurgical procedures.
Americans continue to seek options for enhancing nearly every aspect of the body. In recent years, these options have included everything from procedures to adjust unsightly belly buttons and “hammer toes” to surgeries such as buttocks augmentations and labiaplasty. At the same time, although the majority of patients who opt for cosmetic help are women, the percentage of men who opt for cosmetic procedures has risen well into the double digits, according to Brian R. Buinewicz, M.D., of Buinewicz Plastic Surgery and Le Medspa, which has locations in Doylestown and Flemington, N.J.
Dr. Buinewicz’s practice provides the full range of cosmetic surgical options, including breast augmentation, facelifts and tummy tucks, as well as medically supervised weight loss programs and minimally invasive procedures such as skin tightening, laser treatments and dermal fillers performed by his wife and business partner, Anna Buinewicz, M.D. Although recent advances in cosmetic medicine have led to improved results, reduced treatment times and increased patient safety, it’s only just the beginning; Dr. Buinewicz expects the “next major frontier” in cosmetic medicine to be in areas such as fat grafting—essentially, harvesting a patient’s fat from so-called “problem areas” and transferring it to parts of the body where a patient seeks more volume, such as the breasts.
“Whatever you choose to have done, you really need to find a board-certified physician who is board certified in that particular specialty and who is affiliated with a hospital you trust,” says Dr. Buinewicz. “People do much more research on their new car than they do on their surgeon. But with the Internet, now it’s so easy to check out a surgeon, so you have no reason not to do your homework.”

Although “cosmetic dentistry” is not recognized as a formal specialty by the American Dental Association, we listed dentists according to their particular areas of expertise—oral and maxillofacial surgery, orthodontics, prosthodontics, etc.—that, without a doubt, play a key role in making patients’ smiles sparkle.
